ANKENY COMMUNITY SCHOOL DISTRICT
ACTIVITY PARTICIPATION DOCUMENTATION FOR MINOR'S SCHOOL DRIVER'S
LICENSE
1. Student license requirements for the Ankeny Community School District are:
A. The student is in grades 10-12 at Ankeny High School.
B. The student resides three miles or more from the High School. (Code of Iowa, 285.1.1.b)
C. School transportation is not available to the activity in which the student wishes to be a
participant.D. A certified driver's education program has been successfully completed.
E. The student has had a student learner's permit for at least six (6) months.
F. The District will pursue appropriate action for students who abuse their driving privileges
on school sites or at school activities and encourage the local authorities to provide strict
legal enforcement of the restrictions on student licenses.G. The District reserves the right to deny requests if adequate parking is not available at the High School.
H. Conditions set by the DOT and DOE are met.

A parent and the student must schedule a meeting with the Associate Superintendent (965-9600) to complete the minor's school driver's license application process.
Bring this completed form and the Certificate of Completion of your Driver's
Education Program to the meeting with the Associate Superintendent.
