It may be necessary for students to take medication during the school day. All medication must be brought to the nurse’s office along with a signed permission form, which is available from the school nurse. All medication (prescription and nonprescription medication) must be in the original container in which it was purchased and must include the following information signed by the parent/guardian:
Name of medication
Dosage (amount) of medication to be administered
Date and time for the medication to be administered
Students who are ill or injured are required to see the nurse before leaving. Parent permission must be granted before going home. Student will take the nurse pass to attendance in the main office and sign out on the purple sheet. Parents picking up ill students may do so at the west entrance near the main office.
